What is CARF accreditation?

CARF International is an independent, nonprofit accreditor of health and human services, founded in 1966. Its group includes CARF Canada and CARF Europe, and it reports accrediting more than 9,600 providers.1 Accreditation means CARF’s surveyors have found that your organization conforms to its published standards for the programs you offer.

CARF describes its process as consultative peer review: surveyors are working professionals from the field, and they consult with your staff during the visit rather than simply inspecting.3 Its standards are person-focused, built around the experience and outcomes of the people you serve.4

Which behavioral health programs can CARF accredit?

CARF organizes accreditation into seven program areas, including Behavioral Health and Opioid Treatment Program.2 Within Behavioral Health, the programs most relevant to substance use and mental health providers include:5

  • Withdrawal Management, Outpatient Treatment, Intensive Outpatient Treatment and Day Treatment
  • Partial Hospitalization, Residential Treatment, Inpatient Treatment and Therapeutic Communities
  • Crisis Programs, Sobering Centers, Court Treatment and Office-Based Opioid Treatment
  • Case Management/Services Coordination, Assessment and Referral, Prevention and Health Home
  • Supported Living and Community Housing

Specialty designations such as Children and Adolescents, Criminal Justice, Older Adults and the new Integrated Primary Care designation can be added to the programs you seek.5 Certified Community Behavioral Health Clinics use a separate manual.6

Opioid treatment programs

OTPs use CARF’s Opioid Treatment Program manual and select one core program: withdrawal management, outpatient or residential. Federal OTP certification requires accreditation by a SAMHSA-approved body, and CARF is one of them.7, 8

CARF also delivers ASAM Level of Care Certification for adult residential substance use programs at Levels 3.1, 3.5 and 3.7.9 In California, an ASAM residential certification can satisfy the state’s level-of-care requirement for licensed facilities. See our California licensing guide.

The CARF accreditation process, step by step

CARF publishes its process in plain terms.3 Here it is, with the details that trip up first-time applicants.

  1. Tell CARF who you areSubmit the New Organization Questionnaire. A CARF resource specialist identifies the right standards manual and offers free phone and email support, and you set up a free Customer Connect account.
  2. Evaluate yourself against the standardsCARF suggests starting about 12 months before your survey, with a presurvey using the survey preparation workbook around nine months out, followed by an action plan to close the gaps.10
  3. Apply for your surveyPropose a two-month survey window and submit the application at least three months before it. CARF suggests about six.10
  4. Pay the survey feeCARF invoices the fee, which depends on the number of surveyors and survey days, and then schedules your survey.
  5. Meet your survey teamCARF matches industry-peer surveyors to your programs. You receive written notice of the exact dates at least 30 days ahead, the team coordinator calls about three weeks out, and you display a Survey Notice Poster.
  6. Host the surveySurveyors observe services; interview persons served, staff, board members, funders and referral sources; review documents; and consult with your team.
  7. Receive your decisionThe decision and written report typically arrive six to eight weeks after the survey, and the certificate follows within 60 days.
  8. Submit your Quality Improvement PlanYour QIP is due within 90 days of the decision notice.

CARF accreditation outcomes

OutcomeWhat it means
Three-Year AccreditationAll Accreditation Conditions met, with substantial conformance to the standards
One-Year AccreditationSignificant deficiencies that CARF believes can be corrected
Provisional AccreditationA one-year term after an expired One-Year; the next survey must reach the Three-Year level or the result is Nonaccreditation
NonaccreditationMajor deficiencies, health or safety concerns, or unmet Accreditation Conditions
Inaugural One-Year AccreditationFor new organizations before they begin serving people, with a full follow-up survey about six months after services start

A decision can also carry stipulations that add reporting requirements. Five-year terms exist only for continuing care retirement communities.11

After accreditation: what CARF expects

  • Quality Improvement Plan within 90 days of the decision notice.3
  • Annual Conformance to Quality Report (ACQR) every year; CARF sends the form about 10 weeks before it is due.3
  • OTPs with SAMHSA certification also file a SAMHSA Implementation Report within 180 days.3

Conformance is expected all the time, not only in survey years. That is what our continuing compliance support is for.

What’s new in the 2026 CARF standards

CARF ties each standards manual to a survey year that runs July 1 through June 30. The 2026 Behavioral Health Standards Manual applies to surveys from July 1, 2026 through June 30, 2027, and is sold as the manual, a survey preparation workbook, or both.12 Notable 2026 changes:

  • A first standard on artificial intelligence. Added to all manuals and effective July 1, 2026, it covers written AI policies, disclosure to persons served, data protection, human oversight, incident response and annual review.13
  • Sobering center standards in the 2026 Behavioral Health manual, for surveys from July 1, 2026; CARF plans to fold them into its crisis program standards later.14
  • Integrated Primary Care, a new specialty designation in the Behavioral Health, Child and Youth Services and Opioid Treatment Program manuals, effective July 1, 2026.15

Prepare from the right edition

Because editions follow the survey year, prepare from the manual that matches your survey date, not last year’s copy.

How much does CARF accreditation cost?

CARF does not publish a price list for behavioral health accreditation. Its survey fee depends on the number of surveyors and survey days, and is invoiced before your survey is scheduled.3 Budget separately for the standards manual, staff time to prepare and any consulting support, and ask CARF for a quote once your programs and locations are defined.

Who recognizes CARF accreditation?

CARF reports that governments on five continents recognize at least one of its accreditation areas.1 In the United States, recognition is set state by state and payer by payer. A few examples from our state guides:

  • Florida requires accreditation to renew licensed substance use treatment components, and lists CARF among the accreditors it accepts.
  • New Jersey lets substance use facilities accredited by CARF and other named accreditors apply for deemed status in place of periodic state inspection.
  • Texas law names CARF among the accreditors whose reviews can stand in for state inspections at license renewal.

Check with your state agency and your largest payers before you choose. Our CARF vs. Joint Commission guide covers how to decide.

Common first-time mistakes

  • Starting too late. CARF’s own guidance assumes about a year of preparation.
  • Inflexible schedules. Exact dates arrive only about 30 days ahead, so keep leadership and staff availability flexible within your window.
  • Policies that do not match practice. Surveyors interview staff and persons served and observe services; documents alone will not carry the survey.
  • Treating the decision as the finish line. The QIP is due in 90 days and the ACQR every year.
  • Overlooking new standards. If you use AI tools in services or documentation, written AI policies apply from July 1, 2026.
