CARF survey preparation timeline: from intent to after the survey
CARF says the process may take a year or more of preparation before the survey, and that improvement continues after it.1 Its survey preparation page lays out a month-by-month schedule, which we have combined below with the deadlines that follow the survey.2 If you are still deciding whether to seek CARF at all, start with our CARF accreditation guide.
| When | What to do |
|---|---|
| About 12 months before | Choose the programs to accredit and contact CARF. A resource specialist confirms your standards manual and opens your free Customer Connect account. Form a survey committee, name one member as the CARF contact, send members to CARF 101 training, review the manual and involve leadership and staff at every level.2 |
| About 9 months before | Run a presurvey with the survey preparation workbook to find weak spots, then write an action plan. CARF's resource specialists offer unlimited free help by phone or email.2 |
| About 6 months before | Submit the survey application in Customer Connect, at least three months before your preferred window. Keep preparing documents, locations, staff, leadership, persons served and other stakeholders.2 |
| 3 to 4 months before | Update the materials that show conformance. Decide who can answer questions on each area of the standards, which documents prove each area is in use, and who might be interviewed.2 |
| 1 to 2 months before | CARF sends written notice of the exact dates at least 30 days ahead. Display the Survey Notice Poster. Some organizations run a second self-assessment here.2 |
| About 3 weeks before | The survey team coordinator calls your key contact about plans and logistics. Line up persons served, staff, board members, funders, leadership and referral sources for interviews, with some flexibility.2 |
| 1 week before | Share the general schedule with staff, set up a room for the team, arrange access to files and provide an updated organization chart with names and titles.2 |
| After the survey | A decision and written report in about 6 to 8 weeks, a Quality Improvement Plan within 90 days of the decision, then annual reporting. See the details below.1, 4 |
The months are CARF's suggestions, not deadlines, except the application lead time, the 30-day notice and the post-survey due dates. A program that already runs on written policies can compress the early stages. A program writing its manual from scratch usually cannot.
A step-by-step preparation plan
CARF tells you what to do and when. This is how we turn that schedule into work an owner or clinical director can assign. It is our method, built on CARF's published steps.1, 2
- Lock the scopeList every program, level of care and location you want accredited. Your resource specialist helps determine which manual you need.1 CARF sells the 2026 Behavioral Health publications for surveys conducted July 1, 2026 to June 30, 2027.18
- Build the teamName a CARF contact and a small committee that includes clinical, HR, finance and facilities leads. CARF's own guidance is to involve every level of leadership and staff, not only the quality lead.2
- Run a baseline self-evaluationWork through the survey preparation workbook. CARF says its questions help you judge readiness, find areas to improve and organize materials so the survey team can review them efficiently.10
- Write the action planGive each gap an owner and a date. Expect most of the work to sit in written policies and plans, staff training and the records that prove both. Our policies and procedures service covers the writing.
- Put the documents into practiceTrain staff on every new procedure and let it run for months, not weeks. CARF says conformance is shown through observable practice and verifiable results over time, as well as documents.6
- Apply and pick a realistic windowSubmit the application at least three months ahead (CARF suggests about six) and choose a window your leadership can keep. A 2026 CARF scheduling letter lists a fee for changing dates after scheduling.2, 7
- RehearseTest whether practice matches paper with a second self-assessment or a mock survey: record review, staff and leadership interviews and a walk through each site.
- Organize the weekIndex your evidence by area, confirm interview availability, brief staff and post the survey notice at every location.2, 7
What evidence CARF surveyors look for
CARF's public accreditation reports describe how conformance is judged: through observable practices, verifiable results over time and supporting documents.6 In general terms, the document review can include:
| Evidence type | What it can include | How to prepare |
|---|---|---|
| Organizational documents | Governing and business records: bylaws, policies and plans, financial statements and marketing material6 | One current, approved version of each, with review dates and an owner. Remove drafts and superseded copies from shared drives. |
| Program documents | What each program is, what it delivered and how it evaluates itself6 | A written description for each program that matches what you actually deliver, staff, hours and population. |
| Records of persons served | Charts for people in care now and people discharged6 | Audit a sample of open and closed charts against your own policies before surveyors do. |
| Results over time | Verifiable results over time6 | Collect performance and satisfaction data early enough to show a trend and what you changed because of it. |
| Practice | Surveyors watching day-to-day work and services, and seeing each site in person6 | Walk each site as a visitor would. Check postings, safety items and how staff greet and serve people. |
CARF groups its standards into business practices, called ASPIRE to Excellence, and program standards. ASPIRE stands for assess the environment, set strategy, obtain input from persons served and other stakeholders, implement the plan, review results and effect change.5 Which standards apply depends on your programs, population, locations, service methods and survey type.6
CARF's description of its Behavioral Health manual says it includes appendices on required written documentation and operational timelines, plus intent statements on how to show conformance.9 Those appendices are the fastest way to build a document checklist for your own programs. We cannot reproduce them here because the manual is licensed. Our CARF requirements checklist and CARF consulting page summarize the areas in plain language.
Interviews and observation
Documents alone do not carry a CARF survey. CARF calls the involvement of persons served vital, and its surveyors hold direct, confidential interviews with consenting current and former persons served. Where available, they may also speak with family members, guardians or advocates.6
Interviews with people connected to your organization can include:6
- Leadership: owners, the board and managers.
- Business functions: finance and HR.
- Front-line staff in the programs being surveyed.
- Outside partners: payers and referral partners.
- Community and government contacts.
How to prepare people without scripting them:
- Know the why. Staff should be able to explain, in their own words, why a procedure exists and where it is written down. CARF suggests naming in advance who can speak to each area of the standards.2
- Ask, do not assign. Persons served are interviewed only with consent. Tell people about the survey early and ask who would like to take part.6
- Keep the schedule loose. CARF advises flexibility over a rigid interview schedule.2
- Brief outside stakeholders. A referral source or funder who agrees to talk with surveyors should know the dates and the format.
The survey poster invites outside input
CARF requires you to display its survey notice at least 30 days before the survey, where persons served and other stakeholders can easily see it, at each location under review. The poster tells people when the survey is and how to give input.2, 7 Separately, anyone can send CARF a concern, complaint or compliment about an accredited provider.16
What happens on CARF survey day
CARF describes its surveyors as industry peers who take a consultative rather than inspective approach. The team is matched to your programs by expertise and field experience.1 Surveyors are often employed by organizations with CARF-accredited programs.4 Public CARF documents list surveyors in administrative and program roles, with one assigned to compile the report.6, 7
- Before arrival: a 2026 CARF scheduling letter says surveyors contact you about three weeks before the survey to discuss the agenda.7
- Opening: CARF training material from 2018 describes an orientation conference where the team explains the process and you introduce your programs and staff.8
- The core of the visit: the team observes services, interviews persons served and other stakeholders, reviews documents and consults with your staff.1 Expect a tour of each location surveyed.6
- Closing: the same 2018 material describes an exit conference where the team presents strengths, recommendations and suggestions, and you can ask questions and offer any missing evidence.8
- No decision on site: CARF reviews the findings and issues the decision later.1
Show it while the team is there
CARF's reports say the decision rests only on what surveyors learned during the visit; evidence you did not show, or that fell outside the survey, does not count. CARF can also change a decision, up to revocation, if it later finds the information given was not accurate, truthful or complete.6
CARF also notes that the more organized your documents are, the more time surveyors have for interviews, observation and consultation.2 That consultation is part of the value: surveyors can confirm what you do well and suggest improvements.4
Mock surveys and readiness assessments
CARF builds self-assessment into its own schedule: a presurvey with the workbook about nine months out, and an optional second self-assessment one to two months out.2 CARF also sells a standards conformance checklist for self-evaluation and noting areas to improve.11 A CARF accreditation readiness assessment and a mock survey are two ways to do that work with outside eyes. Our comparison:
| Compare | Readiness assessment (gap analysis) | Mock survey |
|---|---|---|
| Best timing | At the start, around the presurvey stage | Once new procedures have run for a few months, well before the survey |
| Main question | What is missing on paper? | Would the team see what the policies promise? |
| Methods | Document and policy review against the applicable standards | Chart audits, interviews, observing care and touring each site |
| Output | Gap list and action plan | Priority fixes, with time left to make them |
A quick readiness self-check before you book outside help:
- The current manual edition is on hand and matched to your survey dates.
- Every policy is approved, dated and has an owner.
- Staff are trained and can explain each procedure in their own words.
- You have enough performance and satisfaction data to show a trend.
- A chart audit is done and its fixes are in place.
- The survey notice is ready for every location.
- Interviewees are identified and willing.
- A room, file access and an updated org chart are arranged.
A mock survey is run by your team or a consultant, not by CARF, and it cannot predict CARF's decision. CARF does not list one among its steps.1 Its value is finding gaps while there is still time to close them. See how we run mock surveys and continuing compliance, or our CARF accreditation consulting.
Common CARF survey preparation mistakes
- Preparing from the wrong edition. The 2026 Behavioral Health publications cover surveys from July 1, 2026 to June 30, 2027,18 and the manual lists changes from the prior year.9
- Skipping the AI standard. Since July 1, 2026, any program that uses AI in service delivery needs written policies on it, reviewed at least yearly.14
- Charts that break your own policies. Surveyors review records of current and former persons served, so audit a sample first.6
- No data history. Conformance includes verifiable results over time. Data collection that starts a month before the survey shows no trend.6
- Unindexed evidence. CARF notes that organized documents leave surveyors more time for interviews, observation and consultation.2
- A missing poster at one site. It must be up at least 30 days ahead, at every location in the survey.7
- Moving the dates after scheduling. A 2026 CARF scheduling letter says any change or cancellation after scheduling brings a fee plus nonrefundable travel costs.7
- Holding evidence back. What you do not present does not count.6
After the survey: report, decision and Quality Improvement Plan
About six to eight weeks after the survey, CARF notifies you of the decision and sends a written report. The certificate, listing the accredited programs, arrives within 60 days of the decision.1 CARF's public reports include:6
- The decision and the basis for it, plus the programs and locations it covers.
- Your strengths, including any exemplary conformance.
- The sections of the standards applied on your survey.
- Recommendations wherever you did not meet the minimum for full conformance. Every recommendation must be addressed in your QIP.
- Consultation: suggestions from surveyors. CARF says these do not mean nonconformance, and you are not required to address them.
Possible decisions are Three-Year, One-Year, Provisional (only after a One-Year) and Nonaccreditation, with Inaugural One-Year for organizations not yet serving people. An accreditation can also carry stipulations, which may require ongoing reporting.3 Our CARF guide explains each outcome.
- Quality Improvement Plan: due within 90 days of the decision notice, describing actions taken or planned for each area for improvement.1 CARF reviews the plan for completeness and can help you develop it.4
- Opioid treatment programs: a SAMHSA-certified OTP also files a SAMHSA Implementation Report within 180 days of the decision.1 CARF is one of SAMHSA's approved accrediting bodies for OTPs.15
- Annual reporting: your organization reviews new and revised standards each year, and leadership signs the Annual Conformance to Quality Report (ACQR).4 CARF sends the form about ten weeks before it is due.1
Write the QIP straight from the report while the survey is fresh, and assign each recommendation an owner and a date. CARF's reports also encourage you to review notes on consultation discussed during the survey.6 Our continuing compliance support keeps the QIP, data and annual report on schedule.
What a CARF survey costs to prepare for
CARF's public site gives no fee amounts; it asks you to contact it for an estimate. The fee depends on the number of surveyors and days needed, and CARF invoices it before scheduling begins.1 CARF says fees are not based on revenue and add applicable travel expenses and taxes.4 Ask your resource specialist for an estimate once your scope is set. Published prices for the 2026 Behavioral Health materials on November 4, 2026:
| Item | CARF price |
|---|---|
| 2026 Behavioral Health Standards Manual, printed | $2149 |
| 2026 Behavioral Health Survey Preparation Workbook, printed | $13210 |
| Standards Conformance Checklist, printed | $6611 |
| Manual and workbook, electronic download | $580 for one user license; more licenses cost more17 |
| 2026 Accreditation Sourcebook, a guide to the process | $12112 |
Other costs to budget:
- Staff time for the committee, policy writing, training and record audits.
- Training: CARF offers CARF 101 sessions, live and on demand, on preparing for and maintaining accreditation.13
- Rescheduling risk: a fee plus nonrefundable travel costs if you move or cancel a scheduled survey.7
- Consulting support, if you use it. CARF does not list a consultant among its steps.1
Our CARF accreditation guide covers the rest of the accreditation process. If you are opening a new program, see how to open a treatment center for how state licensing and accreditation fit together. Our state licensing guides and licensing consultants cover that side.
CARF survey preparation FAQ
How far ahead should we start preparing for a CARF survey?
CARF's schedule starts about 12 months before the survey, with the application due at least three months before your preferred window.2
What should be ready on the first morning of the survey?
CARF's one-week checklist: staff know the schedule, the team has a room, surveyors can get to the files they need, and an updated org chart with names and titles is ready.2 We also suggest indexing your evidence by area of the standards.
Who will the surveyors interview?
Consenting current and former persons served, and where available their families or representatives. Also leadership and board members, finance and HR staff, direct service staff, and outside stakeholders such as referral sources and payers.6
Is a readiness assessment the same as a mock survey?
No. A readiness assessment comes early and finds what is missing on paper. A mock survey comes later and tests whether daily work follows your written policies. Neither is run by CARF. See how our mock surveys work.
Do we have to act on every surveyor suggestion?
No. Every recommendation in the report must be addressed in your QIP. Consultation is optional and does not mean nonconformance.6
Can we change our survey dates?
Yes, but a 2026 CARF scheduling letter says any change after scheduling brings a cancellation or rescheduling fee plus nonrefundable travel cancellation costs. Confirm the current terms with CARF.7
How is preparing for CARF different from preparing for The Joint Commission?
The two accreditors use different standards and survey methods. Our CARF vs. Joint Commission guide compares them. For Joint Commission survey preparation, see our Joint Commission accreditation and Joint Commission consulting pages.
Vie works with programs nationwide on readiness assessments, policies, mock surveys and survey-week support. See how our CARF consulting works.
Confirm these with CARF before your survey
- Which standards manual and programs apply to you, and which publication year matches your survey dates.1, 18
- Whether the orientation and exit conferences described in 2018 material still run the same way.8
- Any minimum period of serving people under the standards before a regular survey. CARF training material from 2018 described a six-month condition, and new organizations not yet serving people have the Inaugural path instead.8, 3
- A fee estimate for your number of surveyors and days, including travel and taxes, and the current cancellation or rescheduling fee.4, 7
- How many days your survey will run, and how surveyors want access to electronic records and documents.
- The agenda, interview list and site visits, during the surveyor call about three weeks out.7
- Whether your state or payers need anything extra from the accreditation report. See your state licensing guide.
